If you would like to discuss your situation in a confidential setting with a professional, certified Foreclosure Counselor, we are here to help. This will guide you in filling out and gathering the required forms and necessary documentation. Client Intake Form: General information required that we collect by the state. Your information is not shared with any outside agencies or organizations and is kept confidential.
Disclosure Statement: Discusses services offered, funding sources, and organization partnerships.
Counseling Agreement and Privacy Statement: Describes the privacy of your information as well as the counseling agreement.
Authorization to Release Information: Authorizes KLHC to serve as your homeowner counselor and to speak on your behalf.
Credit Report Request: Gives permission to KLHC to obtain a credit report.
Workout Plan Worksheet: Information collected for loan modification.
Household Income & Assets Worksheet: Personal assets owend.
Budget & Expense: Outlines personal expenses.
4506-T: Request for Transcript of Tax Return.
Request for Modification and Affidavit: Required for Making Home Affordable programs.
